OMH SH Requests for Proposals – Long Island and NYC

OMH has tentatively scheduled the release of the following three Request for Proposals for Tuesday August 18, 2015.  All RFPS will be posted on the OMH website and Grants System.

 

  • Supported Housing for Adults with Serious Mental Illness Being Released from State Prison to
    the Long Island Region – $16,124 per unit

The New York State Office of Mental Health (OMH) Long Island Field Office announces this Request for Proposal (RFP) for the development and operation of 14 units of Supported Housing for persons with serious mental illness being released from New York State (NYS) prisons and returning to either Nassau or Suffolk Counties.  A total of four (4) units are designated to serve individuals returning to Nassau County and a total of ten (10) units are designated to serve individuals returning to Suffolk County. This RFP seeks to make two awards, one for ten (10) units in Suffolk County and one for four (4) units in Nassau County.  Eligible applicants may apply and receive awards for either one or both counties.

  • Supported Housing for Adults with Serious Mental Illness with High Needs Request for Proposals Long Island- $16,124 per unit

The New York State Office of Mental Health (OMH) Long Island Field Office announces this Request for Proposal (RFP) for the development and operation A total of 40 units of Supported Housing have been allocated for the Long Island Region. This RFP seeks to make two awards, one award for twenty-five (25) Supported Housing units that are designated to serve high need individuals in Suffolk County and one award for fifteen (15) Supported Housing units that are designated to serve high need individuals in Nassau County. Eligible applicants may apply and receive awards for either one or both counties.

  • Supported Housing for Adults with Serious Mental Illness referred from OMH Psychiatric Centers and Community Hospitals- $15,874 per unit

The New York State Office of Mental Health (OMH) seeks to allocate funding to develop and operate a total of ninety (90) units of Supported Housing designated to serve individuals with a serious mental illness who are referred from South Beach Psychiatric Center, Kingsboro Psychiatric Center, and Creedmoor Psychiatric Center and the Community Hospitals located within these catchment areas.  Three (3) awards of thirty(30) units each will be made for each of the identified catchment area.
