The Health Home Project Team has posted an updated “Health Home Care Management Agency Status” spreadsheet on the Department of Health’s Health Home website in the Medicaid Analytics Performance Portal (MAPP) section under the heading MAPP Access (see link below).
Agencies highlighted in blue and whose status is reported as “Completed” have fulfilled all necessary steps.

Agencies that have not completed the necessary steps are strongly encouraged to do so in order to ensure staff have access to the MAPP.
