Since 1980, The Way Back, Inc. has supported adults with mental illness by providing safe housing and recovery-focused services. Our mission is to help individuals with psychiatric disabilities—such as bipolar disorder, schizophrenia, anxiety, and depression—successfully reintegrate into the community.
Residential Direct Care Counselors play a key role by offering support, education, and supervision to residents. Responsibilities include assessing resident needs, providing crisis intervention, coordinating with service providers, maintaining detailed documentation (daily progress notes and monthly Medicaid services), and facilitating recreational activities.

Positions Available: Residential Direct Care Counselor
Locations: Northport, Patchogue, Rocky Point & Cutchogue
Part-Time weekend evening, weekend overnight, and weekday evening positions are available.
Compensation: $20.00/hr. Overtime rate $30.00/hr.; $500 signing bonus

The counselor is responsible for the support, education, and supervision of 10-14 adult clients located in our Community Residence.
Residential Direct Care Counselor – Key Responsibilities:
1. Support clients with daily living needs and promote independence.
2. Provide supervision and monitor clients’ safety and well-being.
3. Recognize and respond to crises when needed.
4. Assist with medication management, including logging prescriptions and conducting medication counts.
5. Maintain accurate client records and documentation.
6. Implement and review individual service plans in accordance with agency procedures.
7. Communicate regularly with supervisors, families, and service providers.
8. Assist clients with household tasks, room upkeep, and personal responsibilities.
9. Accompany clients to appointments and assist with grocery and petty cash shopping.
10. Help plan meals, shopping lists, and daily activities.
11. Facilitate life skills, recreation, and community participation.
12. Attend required meetings and coordinate Service Plan Review (SPR) meetings.
Skills Required:
1. Ability to complete work accurately and efficiently.
2. Ability to independently organize one’s weekly and/or monthly tasks.
3. Ability to write legibly with grammatical accuracy.
4. Ability to work on diversified duties.
Education and/or Experience Required:
1. High School Diploma with 1-3 years of experience.
2. Previous on-the-job experience in community residential programs or counseling situation.
Additional Job Requirements:
1. Must have a valid driver’s license and insurance.
2. Must successfully complete fingerprinting as required by the NYS Office of Mental Health.
