As many of you probably know, OMH is trying to work with LTSS providers to bring more services into our programs because we are serving more and more challenging people, particularly the aging, medically ill and medically frail, who could benefit from long term services and supports.
To that end, OMH has finalized the long term service and supports (LTSS) survey for OMH housing providers. ACL worked with OMH on the format. It is somewhat lengthy, but is relatively simple to fill out by staff who know the characteristics of your clients.

The survey will be open for responses through Friday, April 26, 5pm.
We hope to have a discussion about the results at our May Symposium on May 7th and 8th in Saratoga Springs so PLEASE SAVE THOSE DATES!
You can have multiple employees complete the survey if they wish, move forward and backward through the survey, and are be able to edit their responses until they have completed all the survey questions.
To maximize the response rate, you will also receive the link for this survey through the OMH Regional Field Offices and the link will lead to the same survey with the same deadline.
