New Health Home standards and requirements for Health Homes, Care Management Providers and Managed Care Organizations have been posted to our website (http://www.health.ny.gov/health_care/medicaid/program/medicaid_health_homes/docs/hh_mco_cm_standards.pdf). These standards and requirements were created by the NYSDOH in collaboration with Health Homes, Care Management Providers and Managed Care Organizations to explain and clarify the roles and responsibilities of each provider (HH, CMA, MCO).
