Announcing an In-Person Plan of Care Training in New York City!
MCTAC is excited to announce an in-person Adult HCBS Plan of Care Training for Health Home Supervisors/ Managers. The presentation will be a full day session, lasting from 10 am until 4 pm on October 9th.
The session will cover HCBS eligibility, HCBS Workflow, required elements for the plan of care; and the method for communicating POC’s to each health plan.
Please note that completing the NYS Community Mental Health Assessment online training modules is strongly recommended prior to attendance.
