The Department of Health posted a new list on June 18, 2015 of Health Home Care Coordination providers that should complete the paperwork to access MAPP. The list can be accessed here –mapp_cma_status (1) . Providers in blue are all set. Providers in white still need to complete the paperwork.
If you are NOT on this list but ARE a Care Coordination provider under a Health Home contract or sub-contract, please contact the Health Home Team directly – tell them the name of the HH with which you have a contract for care coordination/case management services. If you are on the list but your agency name appears in white, you should submit the paperwork. If you are unsure of what to do, contact the Health Home Team directly at the above link.
