We are currently seeking passionate individuals for the position of a Per Diem Residential Counselor in our Bay Shore and Westbury Community Residence Programs. In this crucial role, you would assist individuals in developing independent living skills to increase their self-sufficiency, self-esteem, and self-worth. We provide hands-on training and assist with the activities of daily living as well as provide counseling, behavioral intervention, and crisis management for adults living with severe mental illness.

Responsibilities:

Participate in the planning/evaluation of individual service plans and monitor plan objectives
Hands-on training and assisting residents in the activities for daily living and documenting interaction
Responsible for accompanying residents on recreational outings and travel training exercises
Provide counseling, behavioral intervention, and crisis management
Assist in the day-to-day management of the community residence
Responsible for administration of residence in the absence of the Assistant Administrator
Notify the Assistant Administrator/Residential Administrator of any medical/psychiatric emergency
Provide transportation as needed for shopping, medical appointments, etc.
Write progress notes on residents’ activities on a daily basis
Monitors management of medication
Involved with the cleanliness of the residence
Responsible for maintenance and cleanliness of staff office
Advocate for residents’ benefits
Teach residents the skills needed to gain independence
Coordinate staff contacts with local service providers as needed to assist residents in accessing services and to plan for discharge jointly
Work with support services on behalf of the residents’ best interest
Assist and or model the various techniques required to maintain a clean and healthy living environment
Responsible for other related activities

Education and Experience:

High School Diploma with related experience required
Bachelor’s Degree in Human Services preferred
Physical Requirements: Sits, stands, stoops, squats, walks, and moves intermittently during work hours, may push or pull 10-15 pounds daily, may push or pull up to 50 pounds occasionally.
A valid NYS driver’s license and safe driving record are REQUIRED for this position.

Note: Related experience refers to working with people living with mental illness or other disabilities that require assistance in day-to-day living.

Per Diems are on-call according to their availability and may specify which locations they are interested in.
