Hi everyone,
As mentioned in the Health Home Implementation Webinar on July 23, 2014, the Medicaid Analytic Performance Portal (MAPP) section is Health Home webpage. MAPP is a new performance management system which will provide better tools to the Health Home network to support providing care management for the Health Home population. MAPP is currently in the requirements gathering and development phase. This new MAPP section will provide status updates to MAPP development and expected implementation timelines.
This section will also contain links to surveys requesting feedback regarding specific MAPP use cases or processes flows. Surveys are designed to focus on better understanding existing tracking system process flows, defining system requirements, and understanding current business processes in order to develop the MAPP Portal to better support Health Homes and DSRIP performance management technology needs.
The first survey for development of MAPP Portal is available on DOH MAPP website as shown at the link below. Please complete this survey by August 1, 2014.
