Dear Housing Provider,
OMH is collecting statewide level data on rents and leases of Scattered Site Supported Housing and Apartment Treatment sites and are requesting providers complete the attached survey to assist us in gathering this information.
Deadline: The attached survey should be completed and emailed to Amber Vanderwarker at [email protected] by February 28, 2017.
Instructions: The survey is in excel format and broken into three distinct tables, described below:
- Part One: Agency Information – All agencies complete this section.
- Part Two: Supportive Housing – Agencies with OMH funded Supportive Scattered Site housing complete this section. Agencies list each of their supportive scattered site addresses, then answer the following questions as it pertains to each unit.
- Part Three: Apartment Treatment – Agencies with OMH funded Apartment Treatment housing complete this section. Agencies list each of their Apartment Treatment site addresses then answer the following questions as it pertains to each unit.
