The NYS Department of Health (Department) is pleased to inform you that the Health Home Provider Manual (version 2014-1) has been revised. The new version, Health Homes Provider Manual – Policy and Billing (version 2019-1) is now available for access on the Department’s Health Home Policy and Standards webpage at: https://www.health.ny.gov/health_care/medicaid/program/medicaid_health_homes/policy/index.htm
Providers looking to access the Health Homes Provider Manual through eMedNY’s Provider Manual page will be directed to the Health Homes website.
A great deal of time and effort went in to revising this manual and we hope you find it more user-friendly, up-to-date, and informational.
