Responsible for partnering with adults with substance use disorders in a community based program to identify, access, and maintain linkages with treatment, housing, rehabilitation and community services that will help them maintain stable housing in the community. Working within a person-centered approach, the Service Coordinator develops individualized service plans and teaches consumers living skills essential for their success in living independently and focuses on improving the quality of life for each person served in a way that has meaning to that individual. Position requires daily travel to client housing within Monroe County.

Minimum Qualifications:
Bachelor’s degree in human service related field and 2 years paid human service experience or equivalent combination of education and experience required. NYS motor vehicle license, safe driving record and availability of personal vehicle for work required.

Full-time, 35 hours. Schedule: Monday – Friday, 8:30 AM – 4:30 PM
