Responsible for partnering with clients in recovery from mental illness, chemical dependency, and physical illness to provide comprehensive care management, health promotion, transitional care, patient and family support, referral to community and social support services and to use health information technology to link services in order to support individuals improve their health and wellness, live a self-directed life and strive to reach their full potential. Serves as a point-person for assistance to staff when Program Director is not available.
High School Diploma or equivalent and 10 years paid human service experience or equivalent combination of education and experience. Prior case management or service coordination experience preferred. NYS motor vehicle license, safe driving record and availability of personal vehicle for work required.
Full-time: 35 hours. Schedule: Monday – Friday, 8:30 AM – 4:30 PM.
