Provides medication, physical health and nutrition education and support to residential clients with a mental illness in transition to more independent community living. Acts as point person for client medications within SPSRO program. Provides supervision of clients in managing their medications. Properly logs in client medications and ensures documentation is consistent with medication changes. Partners with assigned clients to assist in getting medical needs met. Provides education and support to clients and their supports about medical issues and needed follow up. Collaborates with external medical providers to facilitate client needs being met. Acts as liaison for unit with pharmacy and doctors when questions arise. Provide 1:1 attention and transportation for medical appointments when client is identified in needing added support. Program is regulated by NYS OMH.
High school diploma and 5 years human services or Bachelors degree in human services or equivalent combination of education and experience. Experience working in an adult residential rehabilitation setting serving clients with mental illness is preferred. NYS motor vehicle license, safe driving record and availability of personal vehicle for work required.
Part-time, 20 hours. Schedule: Wednesday 2:00 PM – 7:00 PM, Saturday and Sunday rotating 8:00 AM – 4:00 pm and 2:00 PM – 10:00 PM shifts.
