Assists clients with a substance use disorder in establishing and maintaining eligibility for financial and medical assistance, and educates clients regarding entitlements available through Temporary Assistance, Food Stamps, Medicaid, SSI/SSD and Medicare. Assists clients in linking to other needed community services including searching for safe affordable housing, legal services, selecting health care providers, etc. Partners with Admissions department to support the clients while in the referral process including referrals to community resources and following up with providers. Ensures agency payment for services including establishing funding at onset of services, developing sliding scale or repayment agreements with client, and following-up to minimize uncollected debt.

Minimum Qualifications:
Bachelor’s degree in human service related field and 2 years paid human service experience or equivalent combination of education and experience. Experience working with public funding sources. NYS motor vehicle license, safe driving record and availability of personal vehicle for work required.

Hours:
Full-time, 35 hours. Schedule: Monday-Friday, 8:30 AM – 4:30 PM. Must be flexible to meet client and program needs.
