Performs evaluation and coordination of admission of referrals for adult mental health programs. Serves as agency point of contact for Monroe County SPOA. Assesses and evaluates individuals who are referred to determine their appropriateness for admission. Coordinates internal admissions/transfers within mental health programs. Communicates outcome of admission decision to referral sources and referred individuals. Ensures the agency occupation goals are met by coordinating flow of referrals and evaluations to maximize the utilization of beds. Provides outreach and information to potential referral sources about East House services.

Minimum Qualifications:
Bachelor’s degree in human service related field and two years’ paid human service experience or equivalent combination of education and experience. Demonstrated ability to provide comprehensive assessment and evaluations of persons with diverse psychiatric diagnoses. NYS motor vehicle license, safe driving record and availability of personal vehicle for work.

Full-time, 35 hours. Schedule: Monday – Friday, 8:30 AM – 4:30 PM.
