Provides counseling, education, training and support to adult clients with mental illness in a community based supported apartment setting in transition to more independent community living. Partners with each client on caseload to develop an Individualized Service Plan that is timely and based on client-identified goals and objectives. Duties also include support to clients, monitoring the self-administration of medication as needed, record keeping, responding to emergencies, providing on-call services to clients as needed, and performing other routine tasks as assigned. Position requires frequent travel to client apartments within Monroe County. Program is regulated by NYS OMH.
High school diploma and 5 years human services work experience or Bachelor’s degree in human services or equivalent combination of education and experience. Experience working in an adult residential rehabilitation setting serving clients with mental illness is preferred. NYS motor vehicle license, safe driving record and availability of personal vehicle for work required.
Full-time, 35 hours. Hours fall between 8:00 AM – 5:00 PM Monday –Friday. Saturday shift from 9:00 AM – 4:00 PM once a month.
