Assists 16 clients diagnosed with a serious and persistent mental illness in a permanent Supported/Single Room Occupancy program (SP/SRO) by providing education, training and support to residential clients to maintain the most independent community living possible. Teaches, supports and assist clients in their responsibilities for cleaning, food and supply purchasing, meal preparation, and safe and sanitary conditions within the living situation. Assist clients to follow through with appointments (e.g. medical, therapy appointments), attend these appointments as needed and assist with treatment recommendations.
High school diploma and 1 year human services or Bachelor’s degree in human services. NYS motor vehicle license, safe driving record and availability of personal vehicle for work required.
Full-time, 35 hours. Schedule: Monday – Friday 11:00 AM – 6:30 PM.
