The Department would like to inform the Health Home Community that The Mary Imogene Bassett Hospital has been designated as a New York State Medicaid Health Home serving members residing in/or receiving services in Chenango, Delaware, Otsego, and Schoharie Counties.
The addition of The Mary Imogene Bassett Hospital officially makes the New York State Medicaid Health Home Program a statewide program covering all 62 New York State counties.
Contact information and network partners for this newly designated Health Home will be posted on the Health Home website shortly.
