Here is the latest version of the Adult Behavioral Health Home and Community Based Services (BH HCBS) Plan of Care template that can be used for individuals enrolled in a HARP who are eligible for BH HCBS. This template incorporates all the Federal rules and regulations required for the Adult BH HCBS Plan of Care. In this latest version we added questions on the setting in which the individual lives under Section 1: “Demographic information”; a Housing Questionnaire Attachment; and questions about education and employment support services under Section 4: “Recommended Behavioral Health Home and Community Based Services (BH HCBS)”.
